Act on immediate risk

Concerns about a relative's care can be distressing, especially when you are unsure what is happening between visits. PDC offers a discussion of care and nursing home concerns and whether a defined investigation might add useful information. The person's wellbeing, wishes, dignity and privacy should guide every step.

Do not wait for a private investigation if someone is in immediate danger: contact the emergency services. Suspected abuse or neglect should be raised with the appropriate safeguarding services. In England, your local council's adult safeguarding team is a relevant route; arrangements differ elsewhere in the UK.

Build a clear account of your concern

Where appropriate, raise the issue with the care provider and keep a record of the response. In England, concerns can also be shared with the Care Quality Commission. Be precise about what you observed, what the person told you and what remains uncertain.

  • Record dates, times and the factual details of relevant incidents.
  • Keep copies of correspondence and note who you spoke to.
  • Include the person's own account and preferences wherever possible.
  • Identify the particular question that previous explanations have not resolved.

Assess what further enquiries could add

A focused review may help organise information and identify factual gaps for a complaint or legal adviser. Explain which organisations are already involved so proposed work can be considered in that context. A private enquiry does not replace safeguarding action, clinical assessment or the regulator's role.

Do not install recording equipment without considering consent, privacy and the impact on other residents and staff. The CQC advises that recording someone's care is a significant decision with possible legal consequences. Discuss an appropriate approach before taking steps that could intrude on dignity or compromise useful evidence.

Questions you may have

Must I gather proof before reporting a safeguarding concern?

You should not delay reporting a concern about abuse or neglect while trying to assemble a private investigation. The appropriate safeguarding team can assess the information you have.

Can you guarantee that an investigation will establish poor care?

No. An enquiry may support, challenge or leave questions unresolved. Its purpose is to establish relevant facts, not assume that a concern is proven.

Can you help me decide where to start?

Yes. Explain the concern and what has already been done. Basic guidance on organising the facts and identifying the appropriate next contact may be enough.
